Is Silvio Berlusconi the first head of state ever to be slapped with a model cathedral?

No. Mainly as Silvio is not a head of state. He's the head of government. the Italian president is the head of state.

Better question still looking for an answer to is - Is there a party which performed relatively worse between elections as the Socialist Labour Party in 2005 General Election and at the By Election in 2009?

In 2005 they got 4,036 votes
In 2009 they got 47 votes

Or a 98.8% drop in their vote (being generous and rounding in a nice way).

Don't know, but there ought to be a word for it. Someone wrote a poem musing on why there was a word (defenestration) for being thrown out of a window, but none to describe (for example) the fate of someone tied to the buffers of the 3:10 from Ealing.
